THE FUSION TEAM:
When the concept of
Fusion Physical Therapy & Sports Wellness
P.C. was conceived, the primary goal was to provide
'exceptional service to our clients and patients that makes a
difference'. This was the basis
for establishing a ‘Team’ oriented approach. The Management felt
that without exceptional Team Players any attempt to surpass the
standard level of patient care/customer service would be impossible
to achieve. With this in mind, any prospective employee of
Fusion Physical Therapy & Sports Wellness
P.C. must meet stringent pre-requisites before employment
is considered. For example, all of our employees are CPR, First Aid and AED (Automatic
External Defibrillator) qualified.

Carolyn S. Mazur MPT, ATC,
ART.
President, Fusion Clinical Team Director.
Carolyn established
Fusion Physical Therapy and
Sports Wellness PC
in 2002. She has extensive hands on training and
experience in the complex and competitive fields of sports and
clinical rehabilitation not only as a clinician but as an
athlete.
Carolyn was a member of The New York Gems Professional Woman's
Tackle Football Team and is an amateur bodybuilder. She
earned her Master’s Degree in Physical Therapy from the esteemed
Rockhurst College in Kansas City, Missouri. In 2000 she
moved to New York to pursue her Doctorate of Physical Therapy at
St. John’s University in Queens, NY. She is presently
enrolled at the prestigious
Boston University
and will complete her Doctorate of Physical Therapy Program in
December 2007.
Carolyn is a member of the American Physical Therapist
Association, as well as, a certified Athletic Trainer and a
member of the National Athletic Trainers Association.
Carolyn is also a fully certified
provider of
Active
Release Technique.

Lacy Lynch MPT, COMT.
Fusion Team Physical Therapist.
Lacy joined the
Fusion Team
as a physical therapist in 2006. Lacy received his Master's of
Physical Therapy degree from Western University of Health
Sciences in Southern California in 2001 and is a Certified
Orthopedic Manual Therapist (COMT). Prior to him establishing a
successful career in physical therapy, Lacy spent six years in
the Air Force where he worked as a translator and was stationed
in Berlin. Upon leaving the Air Force, Lacy went to California
State University at Chico, graduating with a Bachelor's of
Science degree in Biology. Being new to New York City he
enjoys traveling with his wife and exploring this great city.
We wholeheartedly welcome Lacy to our team and are delighted to
have him with us! |

Jennifer Snider BS
Clinical Team Leader, Senior PT Aide.
Jennifer’s calm demeanor,
knowledge, and experience enables her to gently guide our
patients to achieve results beyond their expectations.
Originally from San Francisco, CA, Jennifer moved to New York
City in 2004. While in the “The City by the Bay” she attended
San Francisco State University and achieved her Bachelor of
Science Degree in Biology & Exercise Physiology. She is
currently studying for her
Certified Strength and Conditioning Specialist (CSCS) certification. Jen is an
avid runner and yoga practitioner.
She is an excellent team player,
and
is a great asset to our team. |

Erika Greene
BS Fusion Team
Training Specialist, Senior PT Aide.
Erika is our
Fusion
Team Training Specialist and is responsible for training all new
employees working in the clinic. She is also one of our
Senior PT Aides. With her extensive knowledge and
experience in competitive sports and her commitment to physical
health and wellbeing, Erika has proven to be an invaluable asset
to the Fusion Team.
A native of Brooklyn, New York Erika actively participates in
two team sports - soccer and basketball. Along with her
commitment to sports and a demanding schedule at
Fusion,
Erika attends school full time, currently attending Mercy
College in pursuit of a career as a Physical Therapist. She is
a delight to have on our team and we know she can do anything
she sets her mind to! |

Klarisse Mathis BS
PT Aide.
Klarisse is pursing a career as a
Physician’s Assistant and joined the
Fusion Team
to understand and learn the value of PT and how it interacts
with patients and their rehabilitation process.
Klarisse graduated from George Washington University in
Washington D.C. in 2004. Klarisse was born and raised in
Brooklyn, New York, where she currently resides.
Her
personal interests include dancing and traveling. |

Erin Alex BA
Operations Team Leader, Office Manager.
Erin joined The Fusion Team in November 2005 after moving to New
York City from her native Kansas. As Office Manager, Erin
is in charge of managing and supervising all Operational
Policies and employees from Clinical to Administrative.
Erin graduated from the University of Kansas with
a Bachelor of Arts Degree in History in 2004. While attending
The University of Kansas Erin was an active member of Sigma
Kappa Sorority as well as intramural football, basketball and
soccer. Erin enjoys exploring all that New York City has
to offer from art galleries and museums to the Pickle Festival
and the Saturday morning farmers markets. Erin also enjoys
cooking, reading a good book, or just going for a walk on a nice
day and taking in all the sights with her husband. |
